Please visit our locations page to schedule an appointment. When you call to schedule an appointment, we understand and respect your sense of urgency. Our appointment scheduling staff will make every effort to accommodate you at the earliest time possible. We ask that you plan to arrive 30 minutes early for your first appointment so that you can complete all of the necessary paperwork or prior to your visit, you may complete the new patient registration forms available here. Please bring all insurance information including your insurance card and, if necessary, your HMO referral. As our new patient it would be helpful to orient yourself to our practice and the scope of obstetrical care and management for high-risk pregnant women.
